Life Insurance Smoker Identifier in zzusis

As announced earlier, during the month of February, 2012, there is a Life Insurance Special Re-Enrollment period when employees may be able to obtain additional Life Insurance coverage.

Remember, you can view your current life insurance coverage through zzusis at http://zzusis.wsu.edu/.  Under the “Payroll” section, click on the “Employee Benefits” link, where your Life Insurance coverage is listed on the bottom half of the page.

In that section, you will see the word “Smoker”. If you only have Basic Life Insurance coverage, the field to the right of smoker may be blank. This field has no impact on your insurance coverage.

The “Smoker” field should reflect either a “Y” or “N” if you are carrying coverage under the Supplemental portions of the plan. A “Y” would indicate that you are being charged the smoker premium rates, whereas a “N” reflects non-smoker rates. The default is “Y”.

If zzusis lists you as a Smoker or Non-Smoker (Y or N), and it is incorrect, you may fill out a Non-Smoker Certification Form and send it in to HRS Benefits in Pullman. The form can be found on the HRS webpage at www.hrs.wsu.edu/Benefit+Forms.
