The PEBB Program is offering a new wellness incentive program called SmartHealth. To earn the wellness incentive for 2015, subscribers need to do three things:
- Choose a primary care provider
- Complete your PEBB medical plan’s health assessment
- Start a wellness activity, as identified on www.hrs.wsu.edu/WellnessIncentive
How Do I Take My Plans Health Assessment?
To take your health assessment, please find your plan below, and follow the link. The assessment will take approximately 20 – 30 minutes. Once complete, print the confirmation page of your assessment for your records.
- Group Health’s “Health Profile” – Go to the Group Health member website
- Log-in or register for your account, and then take the new questionnaire under “Health Profile” on the right-hand side of the page.
- Kaiser Permanente’s “Total Health Assessment” – Log in or register to take the Kaiser Total Health Assessment
- The link will take you directly to the plan’s health assessment log-in/registration page.
- Uniform Medical Plan’s “General Health Assessment” – Log in or register to take the UMP General Health Assessment
- The link will take you directly to the plan’s health assessment log-in/registration page.
