The one-time Life Insurance Open Enrollment period has been extended by the State’s Health Care Authority until midnight of December 16, 2016 due to employees state-wide reporting they have been unable to complete their life insurance transactions online or reach a MetLife customer service representative by phone.
The extended open enrollment is for life insurance only! Today continues to be the last day for employees to make changes to the medical/dental plans, enroll in the 2017 FSA or DCAP benefit, or add/delete family members.
MetLife’s website is likely to remain busy. To ensure that your life insurance enrollment or changes are received by MetLife by December 16, 2016, please don’t wait. You have two options:
- Go to MetLife’s website at www.mybenefits.metlife.com/wapebb to create an account. Use PEBB Benefits – State of Washington when asked to enter your company name. Once you’ve enrolled in or made changes to your life and/or accidental death and dismemberment (AD&D) insurance, print your confirmation page and keep it for your records.
- Complete and print the MetLife Enrollment/Change Form. You can mail this form to the address shown on the bottom, fax it to MetLife at 1-859-825-6719, or submit it to HRS who will submit it on your behalf. If you fax your form, please keep a copy of your form or the accepted fax transmission for your records. (MetLife will disregard the enrollment period shown at the top of this form.)
